Pelvic floor muscle training versus no treatment, or inactive control treatments, for urinary incontinence in women


Background

Pelvic floor muscle training is the most commonly used physical therapy treatment for women with stress urinary incontinence (SUI). It is sometimes also recommended for mixed and, less commonly, urgency urinary incontinence.

Objectives

To determine the effects of pelvic floor muscle training for women with urinary incontinence in comparison to no treatment, placebo or sham treatments, or other inactive control treatments.

Search methods

We searched the Cochrane Incontinence Group Specialised Register, which contains trials identified from the Cochrane Central Register of Controlled Trials (CENTRAL) (1999 onwards), MEDLINE (1966 onwards) and MEDLINE In-Process (2001 onwards), and handsearched journals and conference proceedings (searched 15 April 2013) and the reference lists of relevant articles.

Selection criteria

Randomised or quasi-randomised trials in women with stress, urgency or mixed urinary incontinence (based on symptoms, signs, or urodynamics). One arm of the trial included pelvic floor muscle training (PFMT). Another arm was a no treatment, placebo, sham, or other inactive control treatment arm.

Data collection and analysis

Trials were independently assessed by two review authors for eligibility and methodological quality. Data were extracted then cross-checked. Disagreements were resolved by discussion. Data were processed as described in the Cochrane Handbook for Systematic Reviews of Interventions. Trials were subgrouped by diagnosis of urinary incontinence. Formal meta-analysis was undertaken when appropriate.

Main results

Twenty-one trials involving 1281 women (665 PFMT, 616 controls) met the inclusion criteria; 18 trials (1051 women) contributed data to the forest plots. The trials were generally small to moderate sized, and many were at moderate risk of bias, based on the trial reports. There was considerable variation in the interventions used, study populations, and outcome measures. There were no studies of women with mixed or urgency urinary incontinence alone.
Women with SUI who were in the PFMT groups were 8 times more likely than the controls to report that they were cured (46/82 (56.1%) versus 5/83 (6.0%), RR 8.38, 95% CI 3.68 to 19.07) and 17 times more likely to report cure or improvement (32/58 (55%) versus 2/63 (3.2%), RR 17.33, 95% CI 4.31 to 69.64). In trials in women with any type of urinary incontinence, PFMT groups were also more likely to report cure, or more cure and improvement than the women in the control groups, although the effect size was reduced. Women with either SUI or any type of urinary incontinence were also more satisfied with the active treatment, while women in the control groups were more likely to seek further treatment. Women treated with PFMT leaked urine less often, lost smaller amounts on the short office-based pad test, and emptied their bladders less often during the day. Their sexual outcomes were also better. Two trials (one small and one moderate size) reported some evidence of the benefit persisting for up to a year after treatment. Of the few adverse effects reported, none were serious.
The findings of the review were largely supported by the summary of findings tables, but most of the evidence was down-graded to moderate on methodological grounds. The exception was 'Participant perceived cure' in women with SUI, which was rated as high quality.

Authors' conclusions

The review provides support for the widespread recommendation that PFMT be included in first-line conservative management programmes for women with stress and any type of urinary incontinence. Long-term effectiveness of PFMT needs to be further researched.

5. El fármaco en investigación
6. Desarrollo y seguimiento del ensayo.
7. Finalización y cierre del ensayo.

VII Reunión Internacional de Investigación en Enfermería - Forandalus 2014

"El éxito de las enfermeras radica en su capacidad para combinar la alta tecnología con el trato humano, también en la formación e investigación y en la solidaridad" (Kirten Stallkecht, presidenta del CIE). "Las enfermeras actúan sobre la base de pruebas e influyen de manera especial en: los cuidados a VIH/SIDA y a sus familias, la maternidad segura, programas de inmunización, aconsejando a personas en situaciones penosas, en las escuelas, los programa del CINDI (no transmisibles)" (Brundtland, directora de la OMS) Ambas declaraciones coinciden con la celebración de los 100 años del Consejo Internacional de Enfermeras (CIE). El cuidado invisible es el trato humano, el trato atento, seguro, fiable, que empodera a las personas sobre su propia salud y cuidado, que acompaña a las personas enfermas en su situación más vulnerable, que ayuda siempre.

La reunión se celebrará los próximos 30 y 31 de mayo de 2014 en el Hospital Clínico Universitario Lozano Blesa de Zaragoza (España).

International Congress of Rehabilitation Nursing



 
La Escuela Superior de Enfermería de Coímbra (ESEnfC) va a organizar, del 27 al 29 de marzo de 2014, el Congreso Internacional de Enfermería de Rehabilitación, en torno al tema “La persona, función y autonomía: rehabilitar en los procesos de transición”.

Este encuentro lo promueve la Unidad Científico-Pedagógica de Enfermería de Rehabilitación de la ESEnfC, en colaboración con los alumnos de la VI promoción de la poslicenciatura de especialización y de la III promoción del máster, ambos en Enfermería de Rehabilitación.

From March 27 to March 29, 2014, the Nursing School of Coimbra (ESEnfC) is going to organise the International Congress of Rehabilitation Nursing on “The person’s function and autonomy: rehabilitating in transition processes”.
The meeting will be promoted by the ESEnfC’s Scientific-Pedagogical Unit of Rehabilitation Nursing in cooperation with the students of the VI Post-Licentiate Specialisation Course and the III Masters Course, both courses in Rehabilitation Nursing.

Enfermería Basada en la Evidencia (Evidence -Based Nursing)

En los últimos años se han cuestionado muchas prácticas asistenciales, la pertinencia de algunas intervenciones, en definitiva, la efectividad y la eficiencia de la práctica clínica. El modelo de la evidencia científica promueve la utilización consciente, explícita y juiciosa de la mejor evidencia clínica disponible para tomar decisiones sobre el cuidado de cada paciente.

Por eso la fundación Index organiza el curso de enfermería basada en la evidencia (evidence-based nursing) para actualizarse sobre esta temática, el curso esta organizado en cuatro módulos con un total de 200 h. Una gran ventaja del curso lleva a cabo de manera virtual, por lo que se puede tomar a distancia. Además mencionan que los mejores trabajos, producto del curso, tienen la posibilidad de ser publicados en las publicaciones de la hemeroteca cantárida.

Norma Oficial Mexicana: para la práctica de enfermería en el Sistema Nacional de Salud

El 2 de septiembre de 2013 se publicó en el Diario Oficial de la Federación la NOM-019, la cual pretende regular y establecer las características y especificaciones mínimas para la prestación de servicios de enfermería dentro del Sistema Nacional de Salud en México.

Aun que la presente norma oficial no deja claro como modifica o interacciona con otras leyes como la Ley Federal de Profesiones por citar un ejemplo, es necesario revisar esta nueva disposición gubernamental que incide directamente con el ejercicio profesional a partir de este año.
